Please fil out these forms prior to your first visit.

These forms are needed for our staff to process your information at your first visit.  Fill them out to the best of your ability.  If you have any questions please note it on the forms and notify us of them.

Illinois parents do not need to fill out a form. They only need to write a letter and sign it stating they have religious exemption.   Read the state code at the Illinois DHS website http://www.ilga.gov/commission/jcar/admincode/077/07700695sections.html
