Trauma - fall from a height motor vehicle accident, direct blow, worker's compensation Unexplained weight loss Unrelenting pain at rest Evolving neurological deficit suggestive of interveterbral disc pathology, stenosis or tumor Known history of cancer, corticosteriod use, IV drug use, use of blood thinners, and known endocrine diseases Pinpoint bony tenderness over the vertebral spinous process Painless loss of joint play Step defect in spinouse alignment Significant scoliosis observed from the physical exam Patient over age 50 Suspected spinal instability Patient does not respond readily to treatment within two to four weeks
